Dengue fever (Bone Breaking Fever) is a vector borne fever, caused by a virus known as arbovirus which is spread by the bite of mosquitoes (Aedes-aegypti, main vector of the disease). This is also known as Break bone fever, because this fever produces very much pain in bones and joints.

Dengue fever is systemic disease caused by one of four sero-types of virus from the genus Flavivirus, which also includes the pathogens responsible for yellow fever and Japanese encephalitis.

The first known cases of dengue fever occurred in Batavia in 1779, and Benjamin Rush, a prominent American Revolutionary War physician and signer of the Declaration of Independence gave the first detailed description of the disease when it caused an epidemic in Philadelphia in 1780.

Dengue most often manifests itself with the sudden onset of the fever five to eight days after a bite from one of several species of Aedes mosquito, including A. aegyptii, A. albopictus, and A. scutellaris.

After two to four days of headache, fever, and muscle aches, the disease eases for 12 to 48 hours before returning with a skin rash, chills, swollen and reddened eyes, and the severe joint pains that caused Rush to name it “break bone fever,” an eponym that remains in use.

Dengue fever is usually benign and self limited, but a small number of cases will progress to the hemorrhagic form, in which bleeding from multiple organs and mucosal surfaces occurs and for which the mortality is about 5 percent. No vaccine is available for dengue, and there is no effective treatment.

Because the mosquitoes that transmit the disease are day-biting, mosquito netting is of limited usefulness, and the only effective means of control has been to eliminate the Aedes vectors.

Dengue fever is a self-limiting disease. When there is a prevalence of Aedes aegypti and Aedes albopictus together, with circulation of more than one type of dengue virus in a particular area, it gives birth to either Dengue Haemorrhagic Fever (DHF) or Dengue Shock Syndrome (DSS).

Dengue fever virus (DENV) is an RNA virus of the family Flaviviridae; genus Flavivirus. Most are transmitted by arthropods (mosquitoes or ticks), and are therefore also referred to as arboviruses (arthropod-borne viruses).

HOW DOES DENGUE SPREAD?

When a mosquito bites a person who has dengue virus in his or her blood, the mosquito becomes infected with the dengue virus. An infected mosquito can later transmit that virus to healthy people by biting them. Dengue cannot be spread directly from one person to another, and mosquitoes are necessary for transmission of the dengue virus. This is called human-to-mosquito-to-human cycle of transmission.

Initially when mosquito bites a person there is no sign and symptoms for four days. After four days virus multiplies in to blood stream, this condition known as viremia. It lasts for 3- 10 day in which patient presents classical dangue fever symptoms.

Once the mosquito becomes infective, it remains so for life. The virus attacks the genital tract of mosquito and start transmitting in the fully developed eggs at the time of oviposition.

Homeopathy medicines are widely used for prevention of Dengue fever. They are used as per the concept of Genus Epidemicus, given in aphorism 102 of Organon Of Medicine.

What is Genus Epidemicus ?

GENUS EPIDEMICUS is that homeopathy remedy which is found to be a suitable remedy for many cases of that particular epidemic and also act as a preventive remedy for that particular epidemic for majority of people who have not been infected yet.

Dr. Hahnemann gave this concept of genus epidemicus in his book Organon of Medicine in para 102.

Now let us see the aphorism 102-

  • 102

In the course of writing down the symptoms of several cases of this kind the sketch of the disease picture becomes ever more and more complete, not more spun out and verbose, but more significant (more characteristic), and including more of the peculiarities of this collective disease.

On the one hand, the general symptoms (e.g., loss of appetite, sleeplessness, etc.) become precisely defined as to their peculiarities; and on the other, the more marked and special symptoms which are peculiar to but few diseases and of rarer occurrence, at least in the same combination, become prominent and constitute what is characteristic of this malady1.

All those affected with the disease prevailing at a given time have certainly contracted it from one and the same source and hence are suffering from the same disease; but the whole extent of such an epidemic disease and the totality of its symptoms (the knowledge whereof, which is essential for enabling us to choose the most suitable homeopathic remedy for this array of symptoms, is obtained by a complete survey of the morbid picture) cannot be learned from one single patient, but is only to be perfectly deduced (abstracted) and ascertained from the sufferings of several patients of different constitutions.

Footnote.1 – The physician who has already, in the first cases, been able to choose a remedy approximating to the homoeopathic specific, will, from the subsequence cases, be enabled either to verify the suitableness of the medicine chosen, or to discover a more appropriate, the most appropriate homeopathic remedy.


Here in aphorism102, Dr Hahnemann wants to explain that the physician should carefully observe and note the chief complaints of several cases (both sexes, of different age group, and of different constitutions) of that particular epidemic.

Here the physician will find 2 types of symptoms-

  1. General symptoms like malaise, skin rashes, body ache, etc. These symptoms are found in almost all cases of that particular epidemic.
  2. Particular symptoms- rare, uncommon and peculiar symptoms with concomitants.

Now the general symptoms of that particular epidemic becomes the characteristic of that disease. For e.g.  Body pain is noticed in dengue. You can make that general symptom becomes more better by noting it more precisely for e.g. body pain during day time in 1st case, body pain during day only on jar in 2nd case. So this is how your general symptom become more characteristic.

The physician should need to carefully pay attention on particular symptoms also, so that he can reach to more suitable remedy and get towards more suitable GENUS EPIDEMICUS of that particular epidemic.

Merits of Genus Epidemicus in clinic-

  1. It saves time of the physician in finding remedy of that particular epidemic.
  2. Act as a preventive medicine (prophylactic medicine) for particular epidemic for those who have not been suffered by that particular epidemic.
  3. Can be used for majority of the patients.

The most commonly suggested homeopathy medicines for prevention or prophylaxis of Dengue fever are Eupatorium perfoliatum and Arsenic album, but the remedy can change every year.

“the most effective remedy for Dengue Fever is Eupatorium Perfoliatum 200c in the Wet dose which I have prescribed to literally thousands of  patients in Sri Lanka where it is rampant and infects about 1000 patients throughout the Island daily.

The dosage is also very relevant and it is 5ml or a teaspoonful taken every 2 hours on Day 1 and the patient will usually recover in about 4-6 hours after the first dose. The Wet dose is made by inserting 2 drops of the remedy in alcohol or 6 pellets into a 500ml bottle of spring water. ”

CLASSIFICATION OF DENGUE FEVER

– It is of 3 types

  1. Classical dengue fever
  2. Dengue haemorrhagic fever (DHF)
  3. Dengue shock syndrome (DSS)

Classical dengue fever

It is the fever which can affect both the sexes of all age groups.

INCUBATION PERIOD IS- 3 TO 10 DAYS

SIGN AND SYMPTOMS OF DENGUE

Dengue patient has repeated attacks of chills, facial flushing, with non-specific febrile syndrome which may accompanied by maculopapular rashes.

Other symptoms may include –

  1. Headache
  2. Nausea and vomiting
  3. Severe myalgia
  4. Retro orbital pain
  5. Skin Rashes
  6. Constipation
  7. Colicky pain
  8. Abdominal tenderness
  9. Arthralgia of knee and shoulder
  10. Anorexia
  11. Weakness
  12. Sore throat
  13. Mild haemorrhagic manifestations (epistaxis, haematuria)

Fever is usually between 390C-400C. Skin eruptions appear in majority of the cases. The rashes may be reddish, pointed, maculopapular mainly at face, neck and chest. These rashes last up to 2 hours to several days.Fever ends for about 5-7days.

Infection with 1 serotype gives immunity against that particular serotype and partial protection against others.

DENGUE HEMORRHAGIC FEVER

It’s the infection caused by more than one dengue virus (double dengue serotypes). Initially the dengue hemorrhagic fever is also very similar to classical dengue fever but shortly after the fever, symptoms of leakage of plasma appears with hemorrhagic symptoms, which also appears such as gum bleeding, epistaxis, haematuria.

Dengue hemorrhagic fever turns to dengue shock syndrome if left untreated, including persistent headache with nausea and vomiting, restlessness.

There is a persistent decrease in platelet count which causes hemorrhage because platelets are responsible for blood clotting.

DENGUE SHOCK SYNDROME (DSS)

Shock syndrome is a dangerous complication of dengue infection and responsible for high death rates in dangue cases.

It is characterized by fluid loss from blood either by leaking form blood vessels into cellular space or vomitting leads to dehydration.

This fluid loss results in hypovolumia. Due to hypovolumia heart can not supply enough blood to the body tissue which initially affects hearts itself and gradually to the other organs of the body (multi organ failure)  results in a state of shock.

No specific methods are available to predict outcome and progression. Careful fluid management and supportive therapy is the mainstay of management. Corticosteroids and intravenous immunoglobulins are of no proven benefit. No specific therapy has been shown to be effective in improving survival.

SYMPTOMS OF DENGUE FEVER & DENGUE SHOCK

Dengue infection should be considered when patients who live in areas where dengue is prevalent present with a febrile illness together with hemorrhagic manifestations or features of shock. Dengue infection begins as a febrile illness; the fever is accompanied by constitutional symptoms and a characteristic flushing of the skin. Intermittent high-grade fever accompanied by chills and bodyache is a feature. Vomiting, headache, muscular pains, and abdominal pain are common, and patients often feel quite ill.

  • The fever lasts 2–7 days and is followed by a fall in temperature; complications of dengue often take place at this point. Patients who remain ill, despite their temperature returning to normal, are more likely to develop shock. Shock generally occurs on day 3–4 of the illness.
  • Thrombocytopenia  (Decrease count of patelets)is a characteristic finding. Platelet counts below 1, 00,000/mm3  together with a rise in the haematocrit ( ratio of the volume of red blood cells to the total volume of blood) define are charecterstic to dangue haemorrhagic fever. Due to the leakage of fluid from blood vessels increases blood concentration and results in high hematocrit value.
  • This leaked out fluid accumulates in body cavities such as fluid around heart ( pericardial effusion), fluid around lungs ( pulmonary effusion), fluid in abdomen (ascites).
  • In hypovolumic state heart can supply enough blood to the organs results in multisystem failure.
  • Acute renal failure is usually due to hypotension in shock syndrome and is associated with increased mortality.
  • Death is usually due to severe haemorrhage with multi organ failure.

DIAGNOSIS AND INVESTIGATIONS FOR DENGUE

The WHO guidelines on dengue have clearly defined criteria for the diagnosis of dengue shock.

  • Confirmation of dengue infection is by serology or detection of dengue viral material in blood by RT-PCR. Dengue-specific IgG and IgM ELISA is widely used.
  • The test is relatively inexpensive, and becomes positive for IgM antibodies on or after day 5 of the fever. IgM ELISA has a sensitivity of 83.9–98.4% and a specificity of 100%.
  • NS1 ELISA
    The non-structural protein 1 (NS1) of the dengue viral genome has been shown to be useful as a tool for the diagnosis of acute dengue infections. Dengue NS1 antigen has been detected in the serum of DENV infected patients as early as 1 day post onset of symptoms (DPO), and up to 18 DPO.                    The NS1 ELISA based antigen assay is commercially available for DENV and many investigators have evaluated this assay for sensitivity and specificity. The NS1 assay may also be useful for differential diagnostics betw  een flaviviruses because of the specificity of the assay.

  • The article is useful but I wish fpr detailed information for its prevention as well as treatment with Homeo medicines. What I have studied so far Crotalus 200 one dose in the morning for five days followed by Arnica 200 for next five days is recommended for treatment of Dengue Fever. Would somebody put some light on its treatment or preventive medicine to safeguard from this disease. I would appreciate this. Thanking you.

  • Eupatorium Perf 200 is best for prevention. Eupatorium Perf 200, Bryonia 200, Gelsemium 30, Rhus tox 200, Arsenic 30 are best for dengue fever.Crotalus 200, Eupatorium perf 200, Phosphorus 200, Ipecac 200, Millifolium 200 are use for dengue hemorrhagic fever.

    • This is Dr Babar from Lahore, in my experience whoever used these medicines like crotalus, eupatorium phosphorus, china, china sulf etc have not been infected. Today more medicines, rhus tox, eupatorium perf , pyrogenum, ferum phos, china, phosphorus, gelsemium I have treated almost 25 patients with these medicines and they all are now perfect but with the slight issue of weaknes.s

  • Preventive remedies are Crotalis 200 and Eupatorium-Purf 200. Take 5 drops of Crotalis 200 one day and Eupatorium-Purf 200 the next day. INSHAALAH it will proof preventive.

  • Lots of symptoms, rubrics and remedies… that’s one way. OR, you can go straight to the root of the problem, and kill directly the dengue virus (and most viruses) with VRX remedy.
