Anal fissure is a laceration, crack or tear in the vertical axis of the anal canal due to repeated trauma by hard stool.

Causes of anal fissure

The exact cause is unknown. Symptoms usually occur after the stretching of the anus from a large, hard stool.

Risk factors involved

  • Constipation
  • Prolonged diarrhea
  • Multiple pregnancies
  • Crohn’s disease
  • Insertion of rectoscope, endoscope
  • Anal cancer
  • Tuberculosis
  • Immune system disorders

Sign and symptoms of anal fissure

Onset of acute anal fissure is characterized by tearing, cutting, or burning pain during or immediately after bowel movement. A few drops of blood may streak toilet paper or underclothes. Painful anal sphincter spasms result from ulceration of “sential pile” (swelling at the lower end of the fissure). A fissure may heal spontaneously and completely or it may partially heal and break open again. Chronic fissure produces scar tissue that hampers normal bowel evacuation.

Diagnosis of Anal fissure

Anoscopy showing a longitudinal tear and typical clinical feature help establish the diagnosis. Digital examination that elicits pain and bleeding supports the diagnosis. Gentle traction on perianal skin can create sufficient eversion to visualize the fistula directly.

  • Iam troubled with constipation for few months earlier, but later I did observe some thing blocking the anal path and hence hard, pencil stools. I tried carbo veg, nux vomica, sepia etc… but not much relief.

Now a days, I could see anus lil bit coming out (almost an inch) after I pass stools. Is this anus prolapse ? what could be the reason for this blockage ? Pls advise what medicines to take.

    • sounds like hemorrhoids may be causing the problem from what you describe. Hope you have found some relief. If not, check remedies for hemorrhoids, such as hammamelis.

  • An anal fissure is a tear in the lining of the anal canal which causes pain during the bowel movement. It does not lead to serious problems. Most anal fissure can be healed with home remedies within few days or weeks. These are called as an acute anal fissure. Some may take more than 8 to 12 weeks for healing. This is called as a chronic anal fissure. This will require medical treatment. If you have trouble this type of problem you may consult a urologist and get to better suggestion.
