Intercostal neuralgia is usually unilateral, but can also be bilateral. It results from irritation to an Intercostal nerve as it emerges through the vertebral foramen. The most common cause of the irritation is trauma or osteoarthritis change to the cervical or dorsal spine. The pain typically radiates to the front of the chest, where it is interpreted as “breast pain”. “A good history will assist in the differential diagnosis.

A clinical diagnosis of Intercostal neuralgia is made by pressing on the Intercostal spaces along the mid anterior axillary line (located just lateral to the anterior border of the latissimus dorsi muscle). If the patient’s symptoms are due to Intercostal neuralgia, pressure on the Intercostal spaces will elicit marked tenderness at the affected dermatome.
