If Only I Can Live In a World with Less Pain

A case of allergic rhinitis, asthma and hypothyroidism handled with the Case Witnessing Process.

I have an invisible enemy,

Not seen by anyone but felt only by me,

It is merciless, unrelenting, uncaring,

My body also not co-operating,

I cry and cry till I sleep with this suffering,

Only to wake up and say again,

If only I could live in a world with less pain……

The definition of an illness for an individual is multifocal; it may be restricted to his physical symptoms, or his mental/emotional disturbances or his deeper imbalances (subconscious level). However, Homeopathy is a holistic science which offers an answer for imbalances at all these levels. For prescribing a homoeopathic remedy, receiving individualistic understanding of a personality at all levels is a major task which homeopaths go through.

Can we simplify this journey? Can there be a process where we all, as homeopaths, come to one unified understanding and thus find a remedy for an individual? Can this journey be reproducible with ease and scientific understanding? Yes, when we have the Case Witnessing Process (CWP) by our side; then we have a solid foundation to do so.

The Case Witnessing Process (CWP) is completely student friendly, once the basics of its three steps are understood thoroughly. Patients methodically slip into their subconscious, source language, and, in the end, give us the whole remedy pattern. The journey of understanding our patient with CWP has a scientific base of universal three steps in the form of passive, active and active- active. The case witnessing flow is not homoeopathic book centric, instead it is patient centric, where his unique individuality is understood by keeping the patient in the centre and guiding him deeper with what he gives us, what he expresses when given space in the initial passive stage.

The Case Witnessing Process has a check post at every level, whether it is confirming the focus or knowing if we are on the right track, or whether the case is complete, etc. This makes the entire journey more and more systematic and accurate. Such in depth and scientific case witnessing is the key to a unified, unprejudiced understanding of an individual.

The given case was taken during my early ventures of applying this scientific step by step case witnessing technique. As you may know, the whole aim of the CWP is to get the right focus first (Passive) and then confirm it (Active) and explore it until the end (Active- Active), to get to the whole source language out of the patient’s mouth. In this case I intended to do the same.

Mrs. K.F, age 62 years is a patient of allergic rhinitis, asthma and hypothyroidism. She also complains of a frozen shoulder and recurrent stomach upset with loose motions.

(NOTE: The case is edited for brevity. Peculiar sentences are highlighted in italics or bold. My explanations are in plain brackets () and green. HG stands for Hand gestures and D, P stands for doctor and patient respectively. Follow ups are summarized in the end).

  • PASSIVE CASE WITNESSING PROCESS:

D: Tell me about you? Just be spontaneous and tell me whatever you think. It will help me to understand you as person.

P: Basically I have come here for a breathing problem and allergic problem. Get this quite often. Even if I walk or talk to somebody I am uncomfortable. Even if I have anything cold, I get this breathing problem. I am Allergic to strong fragrances and detergent powder. Usually there is a lot of sneezing especially in the mornings.

Second problem is I have hypothyroid; and I have water retention problem especially around my eyes for which I am taking allopathic medicines.

I have this frozen shoulder which is hurting me a lot, can’t lift up my right hand above this (Raises her hand to 40 degree angle and shows).

My digestion is not very strong. I get loose motions and vomiting at same time then my blood pressure goes down leading to dizziness.

(PAUSE)

(We see until now the patient is talking only about her complaints. We passively be with the patient and allow her to go on)

D: Go on, tell more about you as person, your nature, your likes, dislikes everything which will help me to understand your unique personality.

P: I don’t interact a lot. Was very aggressive but now have calmed down. Have gone through terrible times when my parents were not well and my mother was totally bed ridden. Have really gone through hell during last five years (With facial expressions). During previous time I had never fallen sick but I don’t know after my parent’s illness episode I have not been keeping well. Tensed during those periods (Eyes focused) because she (mother) was not keeping well at all, totally bed ridden. Now I am fine, there is no mental hardship or tension that I used to have then.

(Characteristic out of place feelings of tension, hardship and hell when parents were not well have come up. There is a change in her facial expressions while mentioning it and her complaints seem to have started after this whole episode. Hence this becomes important and needs to be noted).

P: I am happy go lucky, no complaints for anything. I am more emotional, in the sense if something happens to somebody and I see it myself, I get very much disturbed. I don’t have that ah, what you say…., I really get panicky and disturbed (Eyes widened). This has come maybe because I have seen many casualties taking place during my parent’s time. My father was unwell for a very long and he used to faint and get hurt; so all these have made my nature …you know… I really get panicky if something happens with somebody. I am really concerned with it, ‘Oh what will happen that way’ (Voice becomes lower). Extreme cases with my father that made me really panicky. What will happen, that uncertainty, anything can happen anytime, have to be vigilant.

(Again she talks about getting disturbed and panicky if she sees something happening to somebody. Here she also generalizes the feeling of getting panicked, which she got during her parent’s illness. Also, while narrating these tense, panicky situations, I observe a change in her voice tone and facial expressions. When the body gets involved along with the verbal language it becomes very important and needs to be noted).

P: If something goes wrong somewhere, I am really concerned with it, “Oh my! what will happen?” that way.

Got a polish and was trying to spray it suddenly after ten minutes I started sneezing and feeling suffocated, slowly I couldn’t breathe and had to take injections from family doctor.

D: Go on, more about you as a person.

P: Someone talking loudly, worldly things I don’t like it, you know how it is. Love to be in company of children, open talks, nothing from behind such company. I read in your article you need dreams but I rarely get dreams, I don’t remember. Can’t go to sleep even if slight noise. I am little reserved in my opinion; won’t tell or react to something immediately. My mind starts working if something goes wrong, if not according to me I start to analyse. That’s all. Now you ask me.

D: You are doing well. Just go with the flow and talk more about yourself.

P: I like people with simple thinking and who are honest. (Repeats)

I was brought up alone and mother was very selective and spiritual. So brought up in a way that you should think before you speak about somebody. Basically I avoid people who are not according to my liking.

I am emotional if I see something on TV, I feel why all this, that’s all (Repeats the sensitivity).

At the end of Passive case Witnessing:

OUT OF PLACE IN PASSIVE:

  • I don’t interact a lot. Was very aggressive but now have calm down.

  • Basically I avoid people who are not according to my liking.

  • I am emotional if I see something on TV feel why all this, that’s all (Repeats this).

FOCUS:

Sensitivity on seeing something happening to somebody came up repeatedly in areas of parents, general and while seeing it on TV. She also connected her complaints to occurring after the parent’s illness.

Another important issue that she kept describing was her dislike of people talking behind her back; also that she likes simple and honest people and that she avoids people whom she doesn’t like. So we still be open and see out of these two what she keeps in the centre. I tell her to summarize, to see what she prioritizes and keeps in the centre.

LEVEL OF EXPERIENCE:Emotional.

(Talks about various common situations, related emotions, tension, panicky, disturbed, and emotional)

LEVEL LOCAL/ GENERAL/ VITAL: General

(In the passive part the patient goes to multiple areas of complaints, parents, general and regarding her nature. So she is describing at general level in the given space of passive.)

HAND GESTURE: Nothing Significant

(Though, Non Verbal language (her change in voice tone and facial expressions) were present while mentioning tense, panicky situations and going through hell during parent’s illness).

IS SHE IN TOUCH WITH HERSELF:Yes

(She mentions issues and talks related to self).

  • ACTIVE CWP WITHOUT FOCUS:

D: Can you summarize about yourself?

P: I don’t like hypocrisy. There is a show and everything is a lie. Makes me very, very uncomfortable and would love to walk out. Want true, simple, honest people.

(Talks about this hypocrisy, again in tune with what she described- ‘people talking from behind’ in passive case, so I ask her to stay with this flow now and see where it goes and what other areas she connects with that to take that as a sure focus).

D: More about this.

P: Say something and do something else, I hate that. Kitty party, functions, were a total waste of time I feel. With some people you can feel that transparency and vibrations.

D: So you don’t like such people?

P: I feel out of place, it suffocates me. So I am not that mixing type, more secluded.

(She had mentioned in passive case- ‘I don’t interact a lot’, which she connects here)

Such people criticize, abuse, show off jewelry. They may admire on face but behind the back they talk something else, no integrity. No substance, rubbish. Purpose of life wasted. No depth.

(Thus, she helps me get the focus by repeating similar ideas about her sensitivity for “people who show off and are different on face and back” and not only that, she qualifies it further. So, I take it as a focus of the patient and inquire further.

The other out of place expression of the patient- ‘Sensitivity on seeing something happening to somebody’ is also very much a part of her centre because it has been qualified in various areas of her life and is connected to her chief complaint. We will see in further journey how this sensitivity gets connected to her whole central state).

  • ACTIVE WITH FOCUS:

D: How does it feel with the Hippocratic people?

(I asked an emotional level question since her level of experience is EMOTIONAL)

P: Disgust Its all drama. When you meet highly evolved people you get positive vibrations. For the former, I have strong hatred, I want to give them back, I get aggressive. I would react, all vocal.

D: What used to happen?

P: They twist and present. I give back vocally. Feel claustrophobic (HG- hands holding the neck). This happens when cruelty happens to you; you want to go away from that. Somebody hurting, somebody killing, you feel like going away.

(While qualifying about the hypocrisy, she also connects the other out of place Sensitivity on seeing something happening to somebody’. There is an appropriate non verbal language (Hand Gesture) coming up. This means ‘the sensitivity on seeing something happening to somebody’ is also definitely part of her central state. We will explore this to get complete understanding of the same).

D: Talk about this cruelty.

P: When somebody is hurt badly; or sight of killing animals; Beating a child, etc. you feel for that person. God save him, you just want to go away. You feel very much hurt, very emotional from within, especially helpless.

(Qualifies by narrating various examples)

D: More about this cruelty.

P: Somebody suffering I cannot see, cannot even forget. Keep coming in front of eyes, feel for that. Feel so sorry, emotional within, Oh why this happened. I put myself in that person’s position and feel-‘oh! How bad’.

It hurts especially when it happens to your own people. When this happened to parents, I was very emotional from inside.

(She connects various issues now, that she spoke about during passive phase of the case- ‘gone through hell when parents were unwell’. So, we know we are on the right track).

P: I cry (teary eyes). I Pray that this should not happen to anybody. I feel helpless; “Oh why!” Especially when I watch my own people or poor people suffering. You feel more and more hurt from within as if it has happened to you. You feel hurt, pain. So much of pain basically. I pray this pain should not be given to anybody.

(We see that the patient qualifies this cruelty and suffering further and she has already started connecting what she had revealed during passive phase of the case. Thus, “Sensitivity on seeing something happening to somebody, cruelty” is a confirmed focus of this case along with the other out of place “don’t like hypocrisy and lie” Also, Non-verbal language is coming up in alignment with the verbal talk. Thus she is ready for the further subconscious journey. This is the best time to start active-active so that we do not encounter resistance from the patient to go deeper. We become Active- Active to bring out the whole altered pattern.).

  • ACTIVE – ACTIVE CASE WITNESSING PROCESS:

D: Tell about this hurt, pain, suffering.

P: If somebody is in pain, I feel emotional, “what if this happens to you?” I feel very sensitive about the suffering the other person is undergoing.

D: What do you feel during suffering?

P: I imagine the suffering. If there is a small hurt, I feel “how much pain!” The Pain that they cannot bear. “Ah!” I get emotional. When my parents were not well, I used to keep on thinking, “what if that… what will happen…etc.”. Mind had no tolerance.

D: More about this cruelty, pain suffering.

P: When you see animals shouting out of agony; when people gets treated during pain, I feel- “Ah!” That hurts you, you become emotional.

D: Tell about this agony.

P: You cannot see the pain. You start thinking if this happens to me how I will feel (Repeats). You have to bear so much. Actual pain (kitna kharab hota hai) (Actually how bad the pain must be)! If this happens to me, I will feel very much hurt from within.

(Here the patient is giving us the quality of pain, what kind of pain she is sensitive to. It’s more of pure sensitivity which she is describing)

  • ACTIVE -ACTIVE TOWARDS WHOLE PATTERN:

D: What happens deep within?

P: Palpitation increases, too much of it, you feel like going for urination. You have tears and your heart beat increases.

D: Stay with this feeling and just this hurt, suffering, pain leading to increase in palpitation and tears.

P: You feel, don’t know how to describe it.

(As she is going deeper she is not able to express that deep non human language).

D: You said you feel claustrophobic?

(Enquiring about the other out of place which came up with non verbal language to completely understand the process)

P: Can’t do anything, suffocated. It’s like stand still, stranded, whole thing comes to an end. No thoughts. Just feel like going away from there to release the suffocation. Go away from that cruelty situation. Since helpless it makes you feel more claustrophobic. When you see your own people suffering, it’s like torture, I cannot stand and feel can’t breathe, can’t think, just want to be out of the whole thing.

(While talking about suffocation, she comes back to her focus of cruelty)

D: Tell about the statements, ‘I don’t like hypocrisy. There is a show and everything is a lie’.

(The other characteristic out of place which needs to be completely explored)

P: Maybe criticizing, abusing, show off jewelry, it just suffocates (HG- Hands holding the neck) Just hate to go with these people. Not very comfortable. Nothing inspires or interests me. Purpose of life is wasted. I avoid going where you are not going to get anything. Strong hatred for such Hippocratic people, I avoid going to parties and social functions because of this. Used to be very reactive and vocal before. Used to be aggressive. Now avoid and try to get happiness, don’t want to be a part of it, instead be yourself.

(In area of hypocrisy, patient is describing pure sensitivity to it, again with a reaction of avoiding going around with such people. There is no other process of someone doing something or me v/s you, stronger v/s weaker coming up for it to hint towards ANIMAL ENERGY. Instead it is pure sensitivity of hatred, feeling disgusted and claustrophobic on meeting such hypocritical people. Nothing else coming up, goes to conscious talks while talking about it, so we bring her back to her focus of ‘Sensitivity on seeing something happening to somebody, cruelty, suffering and pain’ to complete the understanding towards source).

D: Give example of highest torture, suffering and pain.

P: Don’t want to face it again. Pray to God that don’t have to face it. Even when I see on TV it comes, cruelty, murders, national calamities, and people killing mercilessly. Ek current pass hota hai (I shiver with palpitations). I feel nervous when see old people suffering or somebody suffering over the road.

When there are patients in house, you do work but your concentration is not there, you feel what I can do. Even today, if something happens to my own people, I feel helpless, nervous. This happened when my parents were not well, didn’t know what to do. They were going through agony and it was making me nervous.

(Connects her whole experience to where she had started (parent’s illness) in passive phase. We know, even her chief complaint started after parent’s illness. Here, in this case, the circle completes and I end exploring the case here).

UNDERSTANDING THE CASE:

  • REMEDY GIVEN:

KINGDOM:

Throughout the case we see the patient is qualifying the ‘cruelty’ as plain sensitivity and giving us the quality of pain. For her the focus is about the intense pain, suffering and agony a person experiences and emotional feelings of hurt, panicky, disturbed due to the same. She has physical reactions of palpitations, crying, shivering and increased urination along with the sensitivity.

Even while talking about the ‘hypocrisy’ she mentions the hatred, feeling disgust and claustrophobic feelings.

This kind of PURE SENSITIVITY clearly points towards the PLANT KINGDOM’.

FAMILY: PAPAVERACEA FAMILY:Sensitivity to intense pain and suffering.

MIASM: SYCOTIC:

  • Just feel like going away from there to release the suffocation. Go away from that cruelty situation.
  • When you see your own people suffering, it’s like torture, I cannot stand and feel can’t breathe, can’t think, and just want to be out of the whole thing.

  • I avoid going where you are not going to get anything. Strong hatred for such hypocritical people,
  • Used to be very reactive and vocal before. Used to be aggressive. Now avoid and try to get happiness, don’t want to be a part of it, instead be yourself.

[Her coping up with suffering, pain, cruelty and hypocrisy is by avoidance; indicating the MIASM is Sycotic].

REMEDY: PAPAVERACEA FAMILY+ SYCOTIC= SANGUINARIA.

The other bit of the centre:Hatred towards hypocrisy, lie, show off, back biting. With the whole suffocative and claustrophobic feeling it is typical of the nitricums.

SELECTED REMEDY:SANGUINARIA NITRICUM

POTENCY: 200(Since level of emotions is emotional)

DOSE GIVEN: One dose.

ABOUT THE REMEDY:

INSIGHT INTO PLANTS: VOLUME 2

PAPAVERACEA FAMILY:

In a deeper way if this family can be understood as sensitivity to pain; and this goes beyond a purely human situation to a universal feeling. In papaveracea there is no one specific sensation; rather, there is sensitivity to extreme pain or any extreme external condition such as heat/cold/burning.

The main theme of this family is intense suffering, pain, agony, torture, murdered, violence and a hell-like state. Fright and shock of intense pain and torture. And alternation of numbness and pain may be seen.

SANGUINARIA NITRICUM:

  • Dreams of disaster, frightful.

  • Feeling of helplessness and incapacity (As seen in the patient)

COMPLAINTS: It is a good remedy for acute and chronic catarrhs with sneezing.

FROM JAN SCHOLTEN, THE ELEMENTS:

NITRICUMS:

Nitrogen has claustrophobia with suffocative feeling and wanting to get out.

As far as values are concerned, they like to hold on to what they think is important in life. They are torch bearers, even to the point they get a bit too pushy.

They like the good things in life, in company of good friends and family.

They have hypochondria and fear of loss of life. Their own complaints start after they have heard about an illness in the family or after the death of someone who was very close to them.

  • One interesting fact I came across while reading this remedy ‘SANGUINARIA NITRICUM’ from Jan Scholten’s book is that they feel better after urinating when they go through this sensitivity to pain. And in this patient also during the Active- Active toward the source process, she mentions that she feels like going for urination when she feels this sensitivity at its highest intensity.

The patient was on the following ALLOPATHIC MEDICINES:

Abyphylline inhaler, Levocet anti allergic tablet, Thyroxine 50 mg.

FOLLOW UPS:

AFTER ONE MONTH:

After a month when she came for follow up, her complaints of sneezing, breathlessness, frozen shoulder were aggravated since 4-5 days. Also her whole sensitivity to the suffering was predominant. Her dreams showed the feeling of being raped, killed, cheated. This kind of violence is part of Papaveracea family and cheated, raped feeling is covered by the Nitricum part also. This means her original state is aggravated.

O/E: Chest: B/L Wheezing all over chest, Shoulder movement- SQ (40 degree)

TREATMENT: Sanguinaria Nitricum 200 4hrly (to tackle the acuteness of the episode of complaint).

AFTER 6 DAYS OF ACUTE:

She was better in terms of her breathlessness and allergic rhinitis though the frozen shoulder was still bothering her. Disturbing violent dreams also were absent and her whole sensitivity to cruelty and reacting to it, was a little less and she was not getting disturbed as before.

O/E: Chest: Mild wheezing only in upper lobes, Frozen shoulder- SQ (40 degree).

ALLOPATHY: Anti-allergic tablets and inhalers which she was taking every day in high doses were stopped, except for thyroxine.

TREATMENT: So she was kept on frequent repetition to sustain this improvement. She was given Sanguinaria Nitricum 200 every third day.

AFTER A MONTH:

Physically much better in terms of her cold, cough, breathlessness and uncomfortable feeling. Even water retention around eyes and legs due to thyroid had reduced considerably. Digestion improved in terms of acidity and loose motions. She had not taken any inhalers, abyphylline or anti allergic tablets in between this period.

O/E: Chest: Mild wheezing on and off, Shoulder movement 25% better, could take her hand up until 90 Degrees.

ALLOPATHY: Stopped inhalers and anti-allergics completely; continues taking thyroxine 50 mg.

(When I inquired, her sensitivity towards suffering was still prominent)

TREATMENT: Sanguinaria Nitricum 200 every third day (continue this repetition ‘til state is better and there is no potential for the physicals to come back)

AFTER THREE MONTHS:

Better overall. Physically no breathing trouble, sneezing, cold. No digestion problem. Stamina much better, no exhaustion or uncomfortable feeling especially while walking. Shoulder movements better. She mentions that her whole outlook has changed. She used to get disturbed due to sensitivity to cruelty and hypocrisy but feels happy because she was able to overcome it faster. Now she has developed contentment and has feeling of acceptance. These things regarding cruelty, pain and sufferings don’t go on in her mind continuously.

O/E: Chest: B/L Clear, Shoulder movements normal, could lift her hand up.

REPORTS: Her thyroid reports were stable in last three months.

ALLOPATHY: Advised to start reducing Thyroxine 50mg as per the endocrinologist’s advice.

TREATMENT: Sanguinaria Nitricum 200 every third day (Continue same way ‘til consistently better for a few more months, then we will start reducing frequency slowly.)

AFTER SIX MONTHS:

In this span, she continues to improve overall.

PHYSICALLY: No more allergic cold, sneezing, nose dripping. No suffocation, breathlessness. Digestion much better, shoulder movements normal. Thyroid levels normal. No acute episodes in between.

STATEWISE: That sensitivity to cruelty and suffering has really become nil. Now I get affected but can detach myself from it and react appropriately. Feel very nice with no tension and continuous thoughts. Even if see something on TV, I get over it very fast. Have become happy go lucky; that way medicine has really helped.

DREAMS: No violent dreams anymore.

TREATMENT: So we have reduced the frequency of her constitutional medicine gradually to every fifth day and we are monitoring how she copes up at all levels. She is still on homeopathic medication with us.

(As she will continue to consistently improve in her whole state and complaints, we will reduce the frequency of both the homoeopathic remedy and the thyroxine gradually, until her body is completely asymptomatic, her whole sensitivity to pain and hypocrisy is balanced and her body is healthy enough to handle itself without any further medications.).
