A cephalohematoma is a subperiosteal hemorrhage that results in swelling limited in distribution to one cranial bone (i.e., swelling does not cross suture lines). Subperiosteal bleeding is a slow process; therefore, the cephalohematoma may not be evident until several hours after birth. A skull fracture, usually linear, may be an associated finding/ most cephalohematoma are reabsorbed between the ages of 2 weeks and 3 months of age. In some cases, a bony protuberance remains.
Causes of cephalohematoma
It is due to rupture of small blood vessels of periosteal region as a result of prolong labor.
Clinical feature of cephalohematoma
Seen more in primparous mother and during vaginal delivery but is also noticed in cesarean section delivery. It can be bilateral in 15 percent cases. Swelling is confined to the surface of one of the cranial bone and it doesn’t cross the midline. The swelling is firm and sharply demarcated boundaries. Swelling gradually enhances in size, which take several hours to few days, it is larges on 2 to 3rd day.
If cephalihematoma is severe child may develop jaundice, anemia, or hypotension.
Diagnosis of cephalohematoma
Diagnosis is mainly depends upon physical examination or observation; no laboratory tests usually necessary, CT scan or X ray skull can be done if neurological symptoms appear.
Cephalohematoma complications
Anemia and hyperbilirubinemia is seen depending on amount of blood collected in hematoma. Infection of collected blood may take place in infected newborn.
