Myocarditis definition
Myocarditis is a collection of diseases of infectious, toxic, and autoimmune causes characterized by inflammation of the heart.
Myocarditis causes
- Viral – Coxsackie B, but a wide range of enterovirus, echovirus, adenovirus, polio, influenza.
- Rickettsiae – Chlamydia and coxiella (Q fever)
- Toxoplasma – infants and immune-compromised adults
- Trypanosomiasis – T. cruzi (Chagas’ disease of S. America)
- Bacteria – often toxin-mediated, e.g. diphtheria
- Physical – radiation, severe hypothermia
- Drugs and poisons – emetic, carbon monoxide.
Myocardititis symptoms and sign
The symptoms of Myocarditis depend on the degree of myocardial damage. The Clint may be asymptomatic. Nonspecific manifestations of inflammation such as fever, fatigue, general malaise, dyspnea, palpitations, and arthralgias may be present. A nonspecific febrile illness or upper respiratory infection often precedes the onset of Myocarditis symptoms. Abnormal heart sound such as muffled S1 and S3 murmur, and pericardial friction rub may be heard. In some cases, manifestations of myocardial infarction, including chest pain, may occur.
Myocarditis diagnosis
Diagnosis is made on the basis of history and an ECG, which may show a variety of arrhythmias, conduction defects and myocardial injury with ST and T wave abnormalities. An echocardiogram is helpful in diagnosis and shows reduced ventricular contractility.
Endomyocardial biopsy is occasionally necessary to make the diagnosis. This technique is used obtain a small piece or myocardium for histological examination.
In acute myocardititis there is a marked lymphocytic and inflammatory cell infiltrate. Necrosis is myocytes is seen and, often, interstitial fibrosis, later in the disease the histological appearance become less marked and merge into those of congestive (dilated) cardiomyopathy.
