Dr. Surabhi K. Chitre presents a case of urticaria in a boy of ten.

Age: 10 years / Male

Chief Complaint

  • Itching of the whole body
  • <Exposure to dust
  • Swelling of the lips
  • Small eruptions on the body
  • Eruptions with redness
  • No burning, no scaling
  • Catch cold and cough frequently
  • <Change of weather
  • <Exposure to cold climate, taking cold food and drinks
  • <Exposure to dust

Past History

  • Typhoid

Family History

  • Mother: Urticaria
  • Grandfather: HTN, Cancer
  • Grandmother: R.A., Heart Complaints

Physical Generals

  • Desires: Cottage cheese, Potatoes, Chocolates, Sweets, ice-cream, Fruits, Cold drinks and Cold food
  • Perspiration: Face
  • Stool: Nothing specific
  • Urine: Nothing specific
  • Sleep: -Sleeps on abdomen and occasionally on sides –

Sleep refreshing

  • Dreams:
  • Mind: Average in studies

-Restless

-Loquacious

-Naughty

-Hyperactive child

-Loves science

-Fear of dogs

-Fear of dark

-Loves sports

Diagnosis:

Current Medication: Cetirizine, 1 tablet once a day

Repertorial totality Chronic:

Repertorial Totality Acute:

Prescription: 04-08-2016

Based on the totality the patient was prescribed Medorrhinum 30C

  • Medorrhinum 30C

4 drops B.D. for 30 days

  • Antimony Crudum 30C (SOS): For acute episode of urticaria

4 drops B.D. or T.D.S

1st Follow up: 03-09-2016

  • Patient is better
  • Did not observe much change in urticaria
  • Swelling of lips
  • Still takes 1 cetirizine once a day
  • Itching is better

Prescription:

  • Medorrhinum30C

4 drops T.D.S. for 7 days

2nd Follow up: 10-09-2016

  • Urticaria reduced in intensity
  • Frequency of urticaria reduced
  • Swelling of lips is the same
  • Itching better than before
  • No burning

Prescription:

  • Medorrhinum 30C

4 drops T.D.S for 7 days

3rd Follow up: 17-09-2016

  • Itching still present
  • Urticaria intensity reduced
  • Urticaria frequency reduced, earlier it was every day but now comes after every 2 or 3 days
  • Now taking cetirizine every alternate day

Prescription:

  • Medorrhinum 30C

4 drops T.D.S. for 7 days

4th Follow up: 24-09-2016

  • Urticaria frequency much more reduced
  • Urticaria intensity reduced
  • Itching is the same
  • Patient was advised to take half cetirizine every alternate day

Prescription:

  • Medorrhinum 200C
  • drops B.D. for 21 days

5th Follow up: 22-10-2016

  • Patient much more better
  • Itching is better
  • Patient was advised to stop cetirizine

Prescription:

  • Medorrhinum 200C
  • drops T.D.S. for 15 days

6th Follow up: 19-11-2016

  • Patient did not take medicine for 2 weeks
  • Got acute urticaria: was given 5 doses of Antim Crude 30C
  • Tonsillitis 7 days back
  • Frequency of urticaria reduced
  • Intensity of urticaria reduced

Prescription:

  • Medorrhinum 200C
  • drops T.D.S. for 7 days

7th Follow up: 26-11-2016

  • Urticarial around the ankles
  • Patient went to play some sports and was exposed to dust

Prescription:

  • Antim Crudum 30C

4 drops T.D.S for 2 days

  • Medorrhinum 200C
  • drops T.D.S. for 15 days

8th Follow up: 07-01-2017

  • Patient did not take medicine for 2 weeks
  • The frequency and intensity of urticaria is reduced
  • There is no itching
  • Episodes of urticaria were only seen 2 times (A few doses of Antim Crudum 30C were given)
  • Patient has stopped taking cetirizine

Prescription:

  • Medorrhinum 200C
  • drops T.D.S. for 15 days
