Dengue Fever in a Man of 39

Drs. Raj K Pandit and Swati Pandey share a case of dengue fever in a man of 39. Irritability, bitter taste in mouth, fever with chill, desire to lie still were symptoms leading to the simillimum.

Abstract:

Dengue fever, also known as Break-bone or Dandy fever, is an acute viral infection caused by Dengue virus (DENV) harbored by female mosquitoes, Aedes aegypti mainly. In conventional medicine treatment includes, analgesics and sedatives only. We present a case of Dengue fever (Homeopathy for Dengue Fever) with positive findings in blood reports, that was managed homeopathically. The result demonstrates the importance of individualized homeopathic medicine in the management of dengue fever.

Case Report:

A male, aged thirty-nine presented with fever and chill for 5 days. Headache especially on both the temples which was ameliorated by hard pressure. Weakness and pain all over the body which was ameliorated by lying down quietly. Patient had consulted an allopathic physician and was given many doses of antipyretics, but found no relief and platelet counts were decreasing continually.

Past History: H/o jaundice in 2018, treated allopathically.

Family History:

  • Father: H/o HTN & Type II D.M., Mother: H/o O.A., Sibling: No such history.

Physical General:

  • Appetite: Poor
  • Thirsty
  • Tongue: Dry, white coated, with bitter taste.
  • Perspiration: Scanty, non-offensive.
  • Stool: Hard stool, not clear, Constipated.
  • Urine:
  • Sleep: Sound sleep.
  • Thermal reaction: Hot patient, desire to cover when chilly but kicks off covering when he becomes warm.

Mentals:

  • Wants to lie down quietly without being disturbed.
  • Does not want to talk.
  • Irritable.

Clinical Examination:

A. General Survey:

  • Appearance: Thin, tall, Decubitus: of choice, Pallor: Absent, Edema: Absent, Respiration rate (R/R): 22 breaths/ minute, Temperature (Temp.): 1020F, Blood pressure (B.P.): 100/70 mm of Hg, Pulse rate (P/R): 90/minute.

B. Systemic examination:

  • Respiratory system: Vesicular breath sound.

                                    Respiration rate- 22 breaths/ minute.

No added sound.

  • Gastrointestinal system: Liver and spleen-not palpable.

                                           Abdomen- soft.

                                           Normal peristaltic movement

  • Cardiovascular system: Pulse rate- 90/minute.

Blood pressure- 100/70 mm of Hg.

No added sound.

  • Nervous system: NAD.

C. Laboratory investigation:

  • On 07/12/12- Platelet count – 95,000/cu mm. (Figure-1)

– Dengue NS1 Antigen (ELISA) – 59.74. (Figure-2)

  • On 09/12/19- Platelet count 72,000/cu mm. (Figure-3)

Analysis and Evaluation of Symptoms:

Physical General:

  • Thirst:
  • Thermal reaction: Hot patient.
  • Stool: Difficult and hard.
  • Sleep: Sound Sleep.
  • Perspiration: Scanty, non-offensive

Particular:

  • Fever with Chill.
  • Headache during fever.
  • Tongue dry coated, with bitter taste.

Totality of the Symptoms:

  • Want to lie down quietly without being disturbed.
  • Does not want to talk.
  • Irritable.
  • Difficult and hard stool.
  • Tongue dry, with thirst.
  • Bitter taste in mouth.
  • Fever with chill.

Repertorial Analysis:

Repertorisation was done by Hompath software using Kent’s Repertory.

Repertorial analysis showed that BRYONIA ALBA covered 6 symptoms out of 7 and got highest (15) marks. And after consulting with standard homoeopathic Materia medica Bryonia alba was prescribed.

PRESCRIPTION: 09/12/2019-  Bryonia alba 30 -2 doses. OD X 2 days  (Later followed by Bryonia 200)

Patient was advised for Blood Platelet count and asked to report every day.

FOLLOW UP:

11-12-19 Irritability better than before but still wanted to lie down without being disturbed, Headache, weakness and body ache better than before, Appetite same as before, bitterness of mouth decreased, coating of tongue decreased, Stool soft but not clear.

O/E, Temp. 1000F, P/R 80/minute, B.P. 110/70 mm of Hg, R/R 20/ minute.

Platelet count 1,46,000/cu mm.

OD x 2days.

12-12-19 Irritability remained as before, wanted to lie down quietly, Weakness, headache, body ache as before, Stool soft but not clear, bitterness of mouth decreased.

O/E, Temp. 1000F, P/R 80/minute, B.P. 110/70 mm of Hg, R/R 20/ minute.

Platelet count 1,33,000/cu mm.

BRYONIA ALBA

200/2 Doses,

OD x 2days.

14-12-19 Mental wellbeing was good. Patient became active, No headache and body ache.  Weakness decreased. Appetite improved, taste of mouth normal, Tongue normal, no coating, Stool soft in consistency and clear. O/E, Temp. 98.60F, P/R 76/minute, B.P. 120/80 mm of Hg, R/R 18/ minute. Platelet count 2,62,000/cu mm.

15-12-19 All symptoms were improved.
