Psoriasis in a Child of 15 Months

Drs. Raj K. Pandit and Swati Pandey present a case of psoriasis of one month in a child of 15 months. A dry itchy, scaly eruption on her scalp ameliorated by scratching and offensive perspiration were among the symptoms leading to the simillimum.

Chief Complaints:

A 15 months old female child, T.G., was brought by her mother on 13-10- 2018 in OPD, presenting symptoms of dry itchy, scaly eruption on scalp for one month. Itching ameliorated by scratching. Discharge from left ear for 3-4 days. Character of discharge is thin and offensive in nature.

Past History

□ H/O of Diarrhea (Green, watery stool) 3 months before.

Family history

□ Father-H/O CA Throat

□ Grandfather-H/O Hypertension

Personal history

□ Milestones-normal

□ Vaccination-Regular

Physical General

□ Appearance-Looking Dirty, Dark Complexion.

□ Appetite-Good

□ Desire-Sweet, chocolate.

□Thirst-normal

□ Tongue- Moist, clean

□ Sweat-Profuse, offensive

□ Urine-NAD

□ Stool-Clear, Regular, Soft

□ Thermal reaction -Hot (Uncomfortable in Warm weather.)

□ Sleep- Good, Lie on Back

Mental General

□ Very active and Energetic

□ Restlessness

□ Usually put everything into the mouth

□ Playful child

On Examination:

Auspitz sign is positive.

Investigation: Not done.

Miasmatic diagnosis: Psora is Predominant.

Totality of the symptoms:

□ Restlessness.

□ Appearance dirty look.

□ Perspiration profuse and offensive.

□ Hot patient.

□ Desire for Sweet

Repertorial Analysis:

The repertorization was done using RADAR 10.0 by Kent Repertory. The medicine Sulphur got the highest mark. After consulting with materia medica Sulphur was prescribed.

Prescription (13-10-2018)

Rx,

Sulphur 200/2doses, OD

Followed by Placebo for 4 weeks

Follow up (10-11-2018)

Itching mildly decreased, Ear discharge diminished.

Appetite- good, Sleep- sound, Stool-Regular, clear.

No other complaints.

Rx,

Placebo for 8 weeks

Follow up (05-01-2019)

Itching and scales decreased, Ear discharge stopped.

Other general features are normal.

Rx,

Placebo for 8 weeks

Follow up (02-03-2019)

       Scales disappeared, No itching.

Pigmentation started.

Other General symptoms -normal

Rx,

Placebo for 8 weeks

Follow up (27-04-2019)

      The normal color of skin regained

Other General symptoms -normal

Rx,

Placebo for 8 weeks

Follow up (22-06-2019)

No relapse of symptoms.
