Plantar Psoriasis in a Man of 65

Dr. Nishtha Bhatt shares a case of plantar psoriasis in a man of 65. Cough after eating fish was a symptom that helped lead to the simillimum.

A 65 old male came with the complaint of hard skin peeling from feet the last six months. Peeling of skin would occasionally give rise to cuts and wounds which used to burn and bleed. Otherwise, no sign of itching. Occasionally sticky fluid used to ooze out but now it is completely dry. He had taken allopathic treatment for the same but was not satisfied as there was no sign of improvement.

 MIND:

He is a hardworking person, working as a driver. He says “I don’t come in anybody’s way until they irritate me with their words or actions.” He is a short-tempered person who gets agitated by others’ behaviour.

Emotional person. He cannot tolerate wrong or injustice done to anyone. He says his sleep gets disturbed when he is emotionally affected.  His younger brother passed away early in the year 2021 and post that he took all the responsibility of his children.  Major complaints started after his responsibility increased. He is willingly doing everything for his brother’s kids. He said he listens to music when upset and music cheers him.

 PHYSICAL GENERAL:

Appetite- normal

Thirst- 1-2 liters/day

Stool- satisfactory, once a day

Urine- normal

Perspiration- moderate, on exertion

Sleep and dreams- sound in general. Sleep gets affected when emotionally disturbed / not remembered

Thermally- Chilly

Craving- Fish++ but fish causes cough

Aversion- sweets

On observation- Black spots on tongue

Reportorial totality:

  • ANGER about trifles
  • Sleep disturbed after emotional, agg
  • Responsibility increased
  • Music Amel
  • Black discoloration of tongue
  • Cough after eating fish
  • Hard skin peeling off feet

REPERTORISATION

The remedies highest were Lachesis, Merc Sol, Sepia, Arsenic Alb, etc.

The final remedy chosen was Lachesis based on totality.

The differentiating rubrics taken are:

– Cough after eating fish

This is a unique symptom with the only remedy being Lachesis.

Skin peeling reduced.

Feeling overall better

8/7/2022

Skin lesion reduced. No peeling of the skin.

Patient was given placebo for 21 days

29/7/2022

Patient is feeling better.

No new lesions were observed.

No peeling of skin

But existing lesions are as it is.

The patient was asked to stop the medication and carry on with a healthy lifestyle.

 Summary:

  • If in a case you find any unique symptom, consider it important in your prescriptive totality.
  • Do not repeat constitutional medicine in every follow-up. Once the case comes to standstill, repeat the constitutional medicine and later you can think of increasing potency if you don’t see the improvement.
